Fill out the form below, and we will contact you to schedule an in-person or virtual appointment with Dr. Matlock.
Once we receive your information, we will schedule your complimentary virtual consultation, which can be held over FaceTime, phone, or Zoom. The consultation will allow you to share your goals and ask questions.
After your consult, we will create a customized treatment plan to help you achieve your desired results. We will schedule an in-office appointment if you are interested in subsequent steps.
As always, Dr. Matlock and his team will provide the utmost care for you before your procedure, during the operation, and post-op. Our guest services will ensure your needs are tended to — at every step of the journey.
